Extenuating circumstances 2007/08

If an organisation believes that its performance or its ability to submit accurate, comprehensive data has been adversely affected by unforeseen or emergency circumstances outside of its control, where it could not reasonably be expected to have contingency in place to mitigate or remove this effect, it may request that this is taken into consideration in the annual health check.

If you believe that, due to the impact of an extenuating circumstance as defined in the protocol, it may be inappropriate and unfair to assess your organisation using a specific aspect of the published criteria, please complete the proforma below and submit it to your strategic health authority or to the Healthcare Commission as directed in the guidance notes:
